Novo Nordisk launches fixed-price, cash-pay Wegovy subscriptions, taking the branded GLP-1 price war into telehealth subscription channels
On 26 Aug Novo Nordisk formally launched a cash-pay Wegovy subscription plan at a fixed monthly fee, turning telehealth platforms into low-price distribution pipes. The first three partners are Ro, WeightWatchers and LifeMD, with HIMS and Sesame expected to join later.

This structurally extends the margin problem exposed in the Q2 results, when revenue rose 38% but gross margin collapsed from 76% to 64%. The more branded GLP-1 volume HIMS sells, the more it dilutes the blended margin. HIMS is being pushed from product company towards fulfilment channel, and Novo-led pricing squeezes its bargaining power and room to differentiate. Of its two tracks, low-margin branded volume and high-margin in-house peptides plus 503A compounding, the first was already diluting profit and is now squeezed harder by outside pricing, while the second is stuck under an FDA and FTC shadow. This is not a fundamental turning point. In the short term it is a volume positive that brings in more branded Wegovy customers (125,000 orders were shipped in the 6 weeks after branded Wegovy launched in March). But for the core bull debate over whether earnings quality will come through, it is a mildly negative structural signal.
Key evidence
- Novo Nordisk formally launched a fixed-price, cash-pay Wegovy subscription plan on 26 Aug (Managed Healthcare Executive, Medical Economics, AJMC, 26 Aug)
- How it works: eligible self-pay patients get branded Wegovy for a fixed monthly fee through designated telehealth platforms
- Three platforms in the first wave: Ro, WeightWatchers and LifeMD
- HIMS and Sesame are 'expected to join' later, in the words of a Novo executive. HIMS is not in the first wave.
- Injectable pricing tiers: $329 a month on a 3-month plan, falling to $249 a month on a 12-month plan (AJMC)
- Oral tablet pricing tiers: from $289 down to $249 a month

- Through a separate channel, the cash price of oral Wegovy is already down to $149 a month (a promotion running until 31 Aug), rising to $199 afterwards
- Q2 results on 10 Aug: revenue up 38%, but gross margin collapsed from 76% to 64%
- Volume reference: 125,000 Wegovy orders were shipped in the 6 weeks after branded Wegovy went live in March
- The jump in profit has been pushed back to Q4, the key point at which the bull case has to deliver
Catalysts to watch
- Whether HIMS formally joins Novo's list of subscription platforms. Moving from 'expected to join' to actually joining would settle its standing in the channel.
- The Q4 profit jump: the next proof point for the bull case on earnings quality, and the referee of the core tension between fast revenue growth and erosion from both gross margin and compliance costs
- FDA regulatory action on 503A compounding, a key milestone for the high-margin in-house line
- How the FTC litigation shadow develops, which affects compliance costs for the high-margin line of in-house peptides and compounded drugs
Risks
- Structural margin pressure: a subscription model priced by Novo thins the mark-up telehealth platforms can charge. If HIMS joins, it will sell someone else's branded drug at a thinner margin, extending the margin collapse from 76% to 64% already seen in Q2.
- Business model positioning: HIMS is being pushed from product company to fulfilment channel, relying on traffic and customer acquisition efficiency rather than a product premium, while upstream brand owners squeeze its bargaining power
- Two-track strategy under strain: low-margin branded GLP-1 volume dilutes profit, while the high-margin in-house peptides and 503A compounding are caught under FDA regulation and the shadow of the FTC lawsuit. Both legs face clear obstacles to delivering.
- Delayed profit: Q2 already pushed the profit jump back to Q4. If outside pricing keeps limiting room for margin improvement, Q4 will be harder to deliver.
- Channel standing after missing the first wave: Novo chose Ro, WeightWatchers and LifeMD as first partners rather than HIMS, which shows HIMS is not the preferred partner in the branded GLP-1 channel
